This refers to your letter dated October 29, 2008 requesting for a Certificate of Tax Exemption under the provisions of R.A. No. 6938, otherwise known as the "Cooperative Code of the Philippines".
Documents submitted disclosed that Our Lady of the Airways Parish Multi-Purpose Cooperative (formerly Our Lady of the Airways Parish Credit Cooperative) with Tax Identification No. 225-200-933-000, with principal address at Chapel Road corner Ninoy Aquino Avenue, International Airport Road, Pasay City is a primary multi-purpose cooperative transacting with members and non-members; that it is registered with the Cooperative Development Authority with Registration No. J-121-4192-8263 dated April 30, 2003; that a Certificate of Good Standing has been issued in its favor by Mr. Manuel C. Lapena Officer-in-Charge, Cooperative Development Authority, Manila Extension Office on May 21, 2008; that the objectives and purposes for which it is formed are the following:
