| Exchange, Inc.'s ("PSE") forms should not be retyped. | ||||
| 2. | Securities and Exchange Commission — ("SEC" or | |||
| Commission) certified true copy of the following: | ||||
| • | Articles of Incorporation and By-laws and the Certificate of | |||
| Incorporation of the ETF Company and latest amendments | ||||
| thereto; | ||||
| • | Certificate of Increase in Capital Stock and relevant | |||
| Treasurer's Affidavit, if any, of the ETF Company; | ||||
| • | General Information Sheet ("GIS") for at least the past two (2) | |||
