The said clarifications/amendments aim, among others, to address your concerns and queries as raised during the seminars regarding said Circular. You are advised to wait for the release of said clarifications/amendments for your guidance in accomplishing the Mandatory Disclosure Form. Said clarifications/amendments will be released prior to the deadline for submission of the Mandatory Disclosure Form on 31 July 2019. Further, only one (1) copy of your duly filled-out Mandatory Disclosure Form should be submitted.The Mandatory Disclosure Forms are to be submitted to the Enforcement and Investor Protection Department (EIPD) at the Ground Floor, North Wing Hall, Secretariat Bldg., PICC Complex, Pasay City or to the nearest SEC Extension Office.For your information and guidance.
