July 21, 1995DOLE MEMORANDUMTO : All Regional Directors (Except NCR, CAR and Region 9)RE : Amendatory Guidelines on the Sugar Workers Maternity Benefit Program and Sugar Workers Death Benefit Program (Department Order No. 23 and 24, Series of 1992)Pursuant to Republic Act 6982 and its implementing rules and regulations and, in order to improve and expand the access of sugar workers to the maternity and death benefit program under the Act, the following amendatory guidelines are hereby issued.1. ON DOCUMENTARY REQUIREMENTS —In the absence of the required Certificate of Employment, the following substitute documents may support the maternity and death benefit claims:a. Certification of Social Amelioration Program (SAP) Officer —The SAP Officer may certify the employment of the claimant for maternity benefit and deceased sugar worker based on the latest Social Amelioration Bonus (SAB) payroll/payslip/envelope, submitted to the DOLE Regional Office by the millers/planters. Said copy of the SAB payroll may be recopied and attached to the application as supporting document.b. If both Certificate of Employment and SAB payroll are not available, the following documents...
