February 18, 1975DEPARTMENT ORDER NO. 005-75To ensure the orderly registration of employers as required by Section 8, Rule II, Book IV, of the implementing regulations of Presidential Decree 442, otherwise known as the Labor Code of the Philippines, the following guidelines are hereby issued for the compliance of all concerned:1. Every employer shall register his business or workplace with the Regional Office having jurisdiction over the same in a registration form prescribed for the purpose (BLS/ISM Form No. 16). The forms are available at any Regional Office free of charge.2. Period of registration shall be as follows:a. New workplaces, within thirty (30) days before operation or commencement of business. cdprb. Existing establishments, registered or not under Commonwealth Act 104, as amended, within sixty (60) days after the effectivity of this Order.3. Duly accomplished registration forms shall be filled either personally or by mail in quadruplicate with the Regional Labor Office having jurisdiction over the workplace or business establishment.4.
