June 22, 1994DOLE MEMORANDUM CIRCULAR NO. 002-94TO : All Regional Directors (Except those of NCR, CAR and Region 9)RE : Explanatory Guidelines on the Sugar Workers Maternity Benefit Program and Sugar Worker Death Benefit Program (Department Order No. 23 and 24, series of 1992)The following explanatory guidelines are hereby issued in conformity with Department Orders No. 23 and 24, (series of 1992) in order to have clearer and more uniform application of the said rules and regulations.I. On Department Order No. 23 (1992)1. The Maternity benefit of P1,000 shall also be paid to a woman sugar worker who is on leave of absence for a period of six (6) months, or who is unemployed at the time of her delivery, miscarriage or abortion, has rendered at least four (4) months continuous or aggregate service immediately within one (1) crop year prior to said delivery, miscarriage or abortion.2. Certified true copy of latest cash bonus payroll, pay slip envelope or salary payroll signed by the employer shall be submitted by the claimant. The DOLE-WAWD shall verify...
