Who May File and Where (Section 3):
- Any person with direct interest may file petition in person.
- File at local civil registry office where record is kept, or where petitioner resides if impractical.
- Filipino citizens abroad can file at nearest Philippine Consulate.
- Petitions for errors/name changes allowed only once.

Grounds for Change of First Name or Nickname (Section 4):
- Name is ridiculous, tainted with dishonor or extremely difficult.
- New name is habitually and continuously used by petitioner.
- Change will avoid confusion.

Petition Form and Contents (Section 5):
- Affidavit stating erroneous entries and corrections sought.
- Supporting documents: certified copy of entry, at least 2 documents showing correct entry, other relevant documents.
- For name change: publish in newspaper for 2 consecutive weeks, submit certification of no criminal record.
- File 3 copies: civil registrar, Civil Registrar General, petitioner.

Duties of Civil Registrar (Section 6):
- Examine petition and post for 10 consecutive days if sufficient.
- Decide within 5 working days after posting/publication.
- Transmit decision to Civil Registrar General within 5 working days.

Powers of Civil Registrar General (Section 7):
- Can object to decision within 10 working days on grounds: not clerical error, affects civil status, or basis for name change invalid.
- Notify civil registrar, who notifies petitioner.
- Petitioner can seek reconsideration or file court petition.
- If no objection, decision is final and executory.
- For denial, petitioner can appeal or file court petition.

Payment of Fees (Section 8):
- Civil registrar can collect reasonable fees, indigent petitioners exempt.

Penalty Clause (Section 9):
- Violation punishable by 6-12 years imprisonment or P10,000-P100,000 fine, or both.
- Government officials face additional civil service penalties.

Implementing Rules (Section 10):
- Civil Registrar General to issue rules within 3 months in consultation with other agencies.

Retroactivity (Section 11):
- Act has retroactive effect if it doesn't impair vested rights.

REPUBLIC ACT NO. 9048 March 22, 2001 AN ACT AUTHORIZING THE CITY OR MUNICIPAL CIVIL REGISTRAR OR THE CONSUL GENERAL TO CORRECT A CLERICAL OR TYPOGRAPHICAL ERROR IN AN ENTRY AND/OR CHANGE OF FIRST NAME OR NICKNAME IN THE CIVIL REGISTER WITHOUT NEED OF A JUDICIAL ORDER, AMENDING FOR THIS PURPOSE ARTICLES 376 AND 412 OF THE CIVIL CODE OF THE PHILIPPINES Be it enacted by the Senate and the House of Representatives of the Philippines in Congress assembled: Section 1. Authority to Correct Clerical or Typographical Error and Change of First Name or Nickname – No entry in a civil register shall be changed or corrected without a judicial order, except for clerical or typographical errors and change of first name or nickname which can be corrected or changed by the concerned city or municipal civil registrar or consul general in accordance with the provisions of this Act and its implementing rules and regulations.
