Creation of the Municipality of San Isidro
- Section 1: Creates a new municipality in the Province of Davao del Norte called the Municipality of San Isidro.

Seat of Municipal Government
- Section 2: The seat of government of the new Municipality of San Isidro shall be in Barangay Sawata.

Territorial Boundaries
- Section 3: The Municipality of San Isidro is composed of the following barangays:
    - From the Municipality of Asuncion: Sawata, Sabangan, Mamangan, Sto. Niño, Igangon, and Kipalili.
    - From the Municipality of Kapalong: Daudao, Datu Balong, Libuton, Pinamuno, Monte Dujali, Linao, and San Miguel.
- The boundaries are:
    - North: Municipality of Kapalong
    - South: Municipality of Kapalong and Municipality of Asuncion
    - East: Province of Compostela Valley
    - West: Municipality of Kapalong
- Any boundary disputes shall be resolved by the appropriate agency, and the disputed area shall remain under the existing administrative supervision until final resolution.

Plebiscite
- Section 4: The Municipality of San Isidro shall acquire corporate existence upon ratification of its creation by a majority of votes cast in a plebiscite conducted in the barangays comprising the new municipality and in the municipalities of Asuncion and Kapalong within 30 days from the approval of this Act. The Commission on Elections shall conduct and supervise the plebiscite.

Officials of the Municipality of San Isidro
- Section 5: Upon ratification, the President of the Philippines shall appoint the mayor, vice mayor, and members of the sangguniang bayan in consultation with the congressional district representative and the provincial governor. They shall serve until the next local elections following the approval of this Act.

Internal Revenue Allotment Shares
- Section 6: During the first year of corporate existence, the Municipality of San Isidro shall receive the Internal Revenue Allotment (IRA) shares of the barangays mentioned in Section 3 from the Department of Budget and Management. Thereafter, the Municipality of San Isidro and all its barangays shall be entitled to IRA shares pursuant to Section 285 of the Local Government Code of 1991.

Appropriations
- Section 7: The cost of conducting the plebiscite shall be borne proportionately by the municipalities of Kapalong and Asuncion and the barangays.

Repealing Clause
- Section 8: All laws, executive orders, rules, and regulations inconsistent with this Act are hereby repealed, amended, or modified accordingly.

Effectivity
- Section 9: This Act shall take effect upon its approval.

Creation. – There is hereby created a new municipality in the Province of Davao del Norte to be known as the Municipality of San Isidro. SEC. 2.
