Pursuant to the implementation of Republic Act (RA) No. 11213, an Act Enhancing Revenue Administration and Collection by Granting an Amnesty on All Unpaid Internal Revenue Taxes Imposed by the National Government for Taxable Year 2017 and Prior Years with Respect to Estate Tax, Other Internal Revenue Taxes and Tax on Delinquencies and as provided for in Revenue Memorandum Order No. 19-2019, below is the BIR Form for Estate Tax Amnesty: HTcADC

In view of the above, all AABs are hereby advised to accept BIR Form No. 0621-EA and to follow the procedures in Bank Bulletin No. 2018-01 for the updating of the BIR Form in the Forms Code table under the Limited Bank Data Entry System (LBDES) or AABs Bank System authorized by the Bureau of Internal Revenue.
