October 18, 2011BIR MEMORANDUMTO:All Revenue Officials and Employees Concerned    SUBJECT:Addendum to the Unnumbered Memorandum Dated September 2, 2011 Re: Special Leave Benefits for Women Under Republic Act No. 9710 (An Act Providing for Magna Carta of Women) In compliance with the Guidelines on the Availment of Special Leave Benefits for Women under Republic Act No. 9710, the following necessary documents shall be submitted to the Medical, Dental and Welfare Division for proper evaluation: 1. Application for the special leave benefit (Civil Service Form No. 6) signed by the employee and approved by the proper signing authorities.2. Current Medical Certificate filled out by the attending surgeon reflecting the gynecological disorder and surgical procedure done.3. Histopathology Report.For your information and guidance.
