This refers to your letter dated February 10, 2008 requesting for a Certificate of Tax Exemption under the provisions of R.A. No. 6938, otherwise known as the "Cooperative Code of the Philippines".
Documents submitted disclosed that Makati Medical Center Family Credit Cooperative with principal address at Unit 1408 Medical Plaza Makati, Amorsolo cor. Dela Rosa Streets, Makati City is a primary credit cooperative transacting with members only; that it is registered with the Cooperative Development Authority with Registration No. MLa-2788-1013 dated March 13, 1992; that a Certificate of Good Standing has been issued in its favor by Mr. Manuel C. Lapeña, Officer-In-Charge, Cooperative Development Authority, Manila Extension Office; that the objectives and purposes for which it is formed are the following:
